Improving care for complex and long-term conditions

Promoting better prevention, earlier diagnosis and faster, patient-centred care and treatment, we will:

  • Expand screening and awareness of major conditions including, stroke, cardiovascular disease and HIV, and ensure prevention measures like PrEP reach more of those who need them.
  • Ensure fair, safe and high-quality care in every part of Wales, by strengthening national clinical pathways – including for stroke, ophthalmology and cardiovascular health – embedding patients’ rights and reducing regional variation.
  • Require that all care and treatment plans in the Welsh NHS begin with a ‘What matters to you’ conversation, and that all test results are actively communicated to patients within two weeks.
  • Enhance systems and data – improving electronic records, strengthening condition-specific monitoring, and making communication with patients clear and accessible.
  • Develop and strengthen the NHS workforce, by supporting Allied Health Professionals, expanding apprenticeships and placements, and improving access to specialist staff across women’s, children’s and older people’s services.
  • Support people to live well with long-term conditions, by improving access to rehabilitation, expanding multidisciplinary primary care hubs, and promoting tailored support for people with sensory, mobility or complex needs.

We will renew the HIV Action Plan for Wales, with a view to ending new cases of HIV by 2030.
